As a District Health Board we serve one of the largest and most diverse Maaori populations in the country. Achieving Maaori health equity is a key priority for us. Our commitment to this is driven by our desire to acknowledge and respect the Treaty of Waitangi and our belief that if we are serious about achieving health equity for our total population, we must first strengthen our commitment and drive to accelerate Maaori health gain in our community.
Our community health services are embarking on an exciting phase of transformational change to deliver quality health services closer to home. This includes implementing interdisciplinary and integrated approaches to patient and whaanau care. Our innovative programmes include reablement, early supported discharge, rapid response, and hospital in the home. The successful applicant will have a passion for working towards achieving equitable health outcomes for our most complex and high needs patients and whaanau. The ability to develop effective working relationships with key stakeholders in both the primary and secondary settings is essential, as is the ability to work autonomously in a variety of community settings. Strong problem solving, clinical assessment, along with effective communication skills are necessary.
